How to Apply for Arizona Long Term Care System (ALTCS)

Complete an application to start the process

Visit ALTCS Offices (azahcccs.gov) and download the application or call ALTCS toll-free at 888-621-6880

• Another person can act on your behalf during the application process
• You will need to provide documents to show that you meet financial and non-financial eligibility requirements
• You must be determined as needing a nursing home level of care
• Email the completed application to altcsregistration@azahcccs.gov
